Birth Certificate
RequiredOfficial birth certificate from vital records
TIPS:
• Order from the state where you were born
• Hospital certificates are NOT accepted
• Can take 2-4 weeks to receive

• Passport
• Certificate of Citizenship
Photo ID
RequiredDriver's license, state ID, or passport
TIPS:
• Must not be expired
• Name should match other documents
• If appearance has changed, bring supporting documentation
Social Security Card
RequiredOriginal Social Security card for all household members
TIPS:
• Request replacement at ssa.gov if lost
• Free to replace
• Takes about 2 weeks
Proof of Residence
RequiredUtility bill, lease, or official mail showing your address
TIPS:
• Must be recent (within 30-60 days)
• Name and address must be visible
• Can use multiple documents if needed
ALTERNATIVES:
• Lease agreement
• Mortgage statement
• Bank statement with address
Lease or Rental Agreement
Current lease showing rent amount
TIPS:
• Needed for housing assistance
• Should show monthly rent amount
• Landlord contact info helpful
Pay Stubs
RequiredLast 30 days of pay stubs for all working household members
TIPS:
• Include all jobs
• Should show gross (before taxes) income
• If paid cash, get letter from employer
Tax Return
Most recent federal tax return (1040)
TIPS:
• Helpful for self-employed individuals
• Shows annual income
• Can get transcript from IRS if needed
Unemployment Letter
Letter showing unemployment benefits amount
TIPS:
• Get from state unemployment office
• Should show weekly benefit amount
• Include if receiving unemployment
Bank Statements
Last 30 days of statements for all accounts
TIPS:
• Include checking and savings
• Shows assets and deposits
• Can print from online banking
Medical Records
Documentation of disability or medical condition
TIPS:
• Needed for SSI and disability-related benefits
• Include doctor contact information
• Recent records are most helpful
Immigration Documents
Green card, work permit, or other immigration status proof
TIPS:
• Only needed for non-citizens
• Include all household members
• Some benefits available regardless of status
